In the event of an emergency, I authorize the VBS staff and volunteers to secure necessary medical treatment for my child if I cannot be reached immediately. I understand every reasonable effort will be made to contact me or my emergency contact.
I give permission for my child to participate in all Vacation Bible School activities. I understand that while reasonable care will be taken to provide a safe environment, the church, its staff, and volunteers cannot be held responsible for accidents that may occur during normal program activities
I give permission for my child to be photographed or recorded during VBS and for those images/videos to be used in church publications, website and promotional materials.
